No personal identifiable data will be collected, as a study participant, and your anonymity and anonymous data will be protected and securely stored on password-protected servers. You can skip any demographic questions you don’t feel comfortable answering. You can also withdraw from the survey at any time prior to clicking “save” at the completion of the survey. If you agree to participate, your participation will be complete upon finishing the online survey.
There are no known risks from taking part in this study. All information obtained in this study is strictly anonymous. While no personally identifiable information will be collected from you for this study, you will receive a personalized report that will use your name and will be sent to your email address. Once your report has been generated, all identifiable material, such as name and email are deleted from the export file. The results of this research may be used in reports, presentations, or publications, but the researchers will not, nor will they be able to, identify you. In order to maintain anonymity and confidentiality, all data from the online survey will be stored on secure servers which only the primary researchers have password-protected access.
Participation in this study is completely voluntary. It is ok for you to say no. Because the online survey data is anonymous, it will not be possible to identify or eliminate your data from the anonymous data set once you complete the online assessment. There is no cost or payment for your participation in the study, but you will recieive an extensive report that we hope you will find of personal valuable.
